I/we request to change my child’s current attendance arrangements to reflect the arrangement shown under ‘New Attendance’ below. If changing your child’s current permanent attendance to a casual attendance, to meet the requirements of Family Assistance Law and Child Care Subsidy (CCS), it is important your child attends their last day of permanent attendance. CCS will not be paid for any days after the last day your child physically attends and you will be liable to pay full fees for these days.
