I confirm I will be liable to pay fees for the bookings requested on this form as per Ambrose fees policy. To receive the early ‘bee’ rate a completed booking form must be provided to the service before 6pm Friday 13th March 2026.

Fees cannot be in arrears at any Ambrose service & direct debit setup at the vacation care service before bookings are confirmed.

I understand bookings are non-refundable. No cancellations accepted after 6pm Friday 13th March 2026.

I understand there will be no refunds for any absent days if my child does not attend a booked day.

I understand in order to claim eligible Child Care Subsidy (CCS, I must action necessary CWA/Mygov confirmation before vacation care commences, and my child must attend on their first and last day of scheduled care. Children who are absent on their first and/or last day/s may have a percentage of their CCS entitlement withheld by Services Australia and therefore be required to pay full fees.

I understand if my child is attending a different vacation care service to the before and after school care service my child attends during Term as indicated on this booking form, I agree the enrolment records for my child can be copied to ensure it is available at the vacation care service specified on this form. If any information relating to my child requires updating, I acknowledge I will advise both the before/after school care service and vacation care service. The enrolment records include but not limited to information held in Xplor, enrolment form, birth certificate, immunisation status, emergency contacts, medical conditions including medical management plans and behaviour plans. The enrolment records will be held by the vacation care service as specified in the Education and Care Services National Regulations.

I understand if my child attends a different before and after school care service and requires medication for an allergy, asthma or medical condition, it is my responsibility to provide the vacation care service with this medication and if required updated medical management plans or dosage requirements as per the Ambrose policy. I will provide the medication on or before the first session and collect it on the last session.

I agree to abide by all policies and procedures of CDPSL and/or Ambrose Early Education & Care.

I acknowledge that children will be supervised at all times and all travel between the service and excursion venue will be in privately booked buses with seatbelts where fitted.

I understand excursions are compulsory for all children booked that day to attend.

I understand “Excursion and Workshop Risk Assessments” have been completed and available upon request.

I understand the ratio’s Ambrose adheres to is 1:15 for in-centre days and 1:10 for all excursions (water-based excursions are 1:5). Additional educators will be rostered where it is deemed to be necessary, based on the activity risk assessment.

I accept it is my responsibility to ensure we are at the service by 8:00am on excursion days (unless another time is specified). I understand the bus will not wait, and I will need to make alternative care arrangements for my child on that day. My child will be marked absent and I will be liable to pay for the booked day.

I understand I am required to sign my child in and out of the program each day. Written notification is required to allow another person to collect my child if they are not listed as an authorised nominee.

I understand my children may be required to participate in activities that may be physically and emotionally demanding. My children’s participation in any activity is not compulsory. CDPSL & Ambrose staff will duly exercise their Duty of Care.

I agree to support the service in implementing the Sun Protection Policy by my child/ren wearing Sun-safe clothing with sleeves & collar, sun-safe hat and sports shoes & socks.

I understand all programs will continue in the event of inclement weather unless notified. I understand should the scheduled venue need to change due to inclement weather, Ambrose will offer a suitable replacement or an in-centre/workshop program.

I understand CDPSL and Ambrose will take no responsibility for lost property. This includes electronic devices.

I understand at times it is out of CDPSL & Ambrose control if a workshop and/or excursion is cancelled. A replacement workshop and/or excursion may be offered for that day. No refunds will be provided if a workshop and/or excursion is replaced, and your child no longer wants to attend the replacement. In the event a workshop and/or excursion is cancelled entirely, the difference in cost between the original program and the replacement program for that day will be applied as a credit on your fee statement in Xplor.
